It is the commonest confusion in the body surgery consultation, and the one that produces most disappointment: thinking liposuction is a fast way of doing what the diet did not. It is not, and it is worth understanding why before paying for it.
What it removes and what it does not
Liposculpture works on subcutaneous fat: the fat between skin and muscle, the fat you can pinch. It does not touch visceral fat, the fat surrounding the organs inside the abdomen, which is what weighs most on the scales and matters most for health. That only comes down with a calorie deficit and exercise.
That is why liposculpture changes the shape a great deal and the number on the scales very little. Someone can leave with their contour transformed having lost two or three kilos. It is an operation about silhouette, not weight.
The part nobody explains: the cells that remain
Removing fat removes fat cells from that area, and those do not come back. But the ones that remain — and they do remain, because an area is never emptied completely — keep their capacity to grow intact.
If the weight rises significantly afterwards, those cells enlarge and the result is lost. And there is an unpleasant nuance: because fewer cells remain in the treated area, the gain distributes differently and sometimes accumulates more noticeably in places that did not stand out before. It is not that the fat "moves"; it is that the proportion changed.
Liposculpture does not maintain your weight. You maintain your weight, and liposculpture sculpts what is left.
What a good indication looks like
- Stable weight for at least six months, and close to what you can sustain.
- Localised fat that does not respond to diet or exercise: flanks, inner thighs, upper back, the area under the chin. Areas with a distribution that is genetic rather than habitual.
- Good skin quality and retraction. This is decisive: removing fat from under skin that no longer retracts leaves laxity, and then what was needed was not a liposuction.
- Expecting a shape, not a number. If what you want is to weigh less, the operating theatre is the wrong road.
And high definition
High-definition liposculpture goes a step further: rather than emptying the area evenly, it leaves a thin layer where it should and removes it where the muscle should show through, respecting the body's natural shadows. It is longer, more dependent on planning and more demanding of the starting point.
Precisely for that reason it is less tolerant of weight changes. Defined relief blurs with fewer kilos than it would take to spoil a conventional liposuction. Anyone unwilling to hold their weight will get a better long-term result from a more conservative goal.
What is not promised
No figure on the scales is promised, nor that cellulite will disappear — it is not fat, it is a matter of septa and skin — nor that stretch marks will improve. Nor that the result is unchangeable: what you do with your weight afterwards counts as much as what was done in theatre.
What it does do, in the right indication, is change a shape that was not responding to anything. Which is not little, but it is something else.
