Indicative figures. Your own surgical and recovery times are set in the consultation, according to the technique indicated.

Surgery is performed in accredited clinics and hospitals in Lima, with the team and facilities each case requires. The consulting room is where you are assessed, where the plan is made and where follow-up happens.

The procedure

Buttock augmentation with your own fat takes tissue from other areas by liposuction and grafts it into the buttock. It gives volume and shape with your own material, and it demands a strict safety protocol.

The technique has a double logic that explains its popularity: fat is removed from where there is too much — abdomen, flanks, back — and placed where there is too little, so the change is not only one of volume but of proportion. The fat is your own tissue, there is no rejection and no foreign body, and the result, where it takes, is permanent.

Now the part that has to be said first and clearly: performed badly, this procedure is the aesthetic operation with the highest mortality described in the literature. The cause is fat embolism, which happens when fat is injected inside the gluteal muscle and enters a large vein. That is why there is an international consensus which is followed here without exception: the fat is placed exclusively in the subcutaneous plane, above the muscle, with a blunt large-bore cannula kept constantly moving. Never intramuscular. That single rule is the difference between a safe procedure and one that can cost a life.

It usually helps when there is

  • Enough fat in other areas to be able to graft it.
  • Wanting to improve the shape and projection of the buttock with your own tissue.
  • Stable weight for at least six months.
  • Expecting a proportionate result and not an extreme volume.
  • Good general health and not smoking.

Worth discussing first if

  • Being very slim, with not enough fat to move.
  • Unstable weight: grafted fat behaves like the rest of the body's.
  • A history of thrombosis or of clotting problems.
  • Expecting a disproportionate volume, which forces risky techniques.
  • Previous biopolymers in the area, which change the whole approach.

None of these rules the procedure out on its own; all of them change the conversation.

Options

Fat, implant or reshaping

The choice depends on the fat available, on the starting shape and on the volume sought.

Graft of your own fat

The technique of choice. It improves shape and projection and also defines the waist by removing fat from the flanks. Part of the graft is reabsorbed.

Buttock implant

Considered in very slim people with no fat available. It gives central projection but has more local complications and a more uncomfortable recovery.

Reshaping without a graft

Sometimes what is missing is not volume but contour: treating the waist, the flanks and the infragluteal fold makes the buttock look more projected without adding anything.

Surgical judgement

The subcutaneous plane is not negotiable

The entire safety of this procedure rests on where the fat is placed. It is injected exclusively above the gluteal muscle, in the subcutaneous plane, with a blunt large-bore cannula, in continuous movement and never pointing the tip towards the depths. The large gluteal veins are inside and below the muscle; if fat enters one of them, it travels to the lung. There are international recommendations on this and they are followed here without exception, even when that limits the volume that can be placed.

The amount grafted has a ceiling: the skin and the tissue only accept a certain volume before the pressure compromises the survival of the fat. Going beyond it does not give more result; it gives more reabsorption, more fat necrosis and more nodules.

And a warning that in Peru is not theoretical: biopolymers, oils and industrial filler substances are still injected into buttocks, often by people with no medical training. They produce chronic inflammation, deformity, migration of the material and, in some cases, systemic illness. They have no antidote and removing them is complex surgery with a partial result. None of those substances is injected here, and there is no circumstance in which they should be accepted.

Stages

What the process is like

From the first consultation to discharge, with the usual timings for this procedure.

Consultation

The fat available, the shape of the buttock and the proportion sought are assessed. The safety protocol is explained, and why it limits the volume.

Before surgery

Laboratory tests and anaesthetic assessment. Thrombosis risk calculated. Stopping smoking. The informed consent is signed.

Surgery

Under general anaesthesia and with antithrombotic prophylaxis. Liposuction of the donor areas, processing of the fat and grafting in the subcutaneous plane with a blunt cannula.

Follow-up

Reviews at one week, one month and the third month, with massage and drainage of the liposuctioned areas as directed.

Timings

Week by week

Not sitting directly on the buttock in the first weeks is what decides how much fat survives.

  1. Days 1 to 14

Compression garment, no sitting directly on the buttock — a cushion is used, or the weight is taken on the thighs — and no sleeping on your back. Walking from the first day.

  2. Weeks 3 to 4

Back to office work at around fifteen to twenty-one days, with a cushion. Sitting for short periods. Massage of the liposuctioned areas.

  3. Weeks 6 to 12

Exercise built back up once authorised. Part of the initial volume is lost: between thirty and fifty per cent of the fat is reabsorbed.

  4. Months 3 to 6

Final result, with the fat that took now settled. This is the point at which to consider whether a second session is needed.

Risks, and what is not promised

The serious risk of this procedure is fat embolism, which can be fatal. It happens when fat is injected in the intramuscular plane and reaches a large vein. It is prevented by a single, absolute measure: grafting exclusively in the subcutaneous plane, with a blunt large-bore cannula, kept moving. Deep vein thrombosis and pulmonary embolism are also serious risks, prevented with prophylaxis and early mobilisation.

Local risks: reabsorption of between thirty and fifty per cent of the grafted fat, asymmetry, irregularities, fat necrosis with palpable nodules, oil cysts, seroma and infection. In the donor areas, those of any liposuction.

On biopolymers: if you are offered a buttock augmentation with injections in a consulting room, with no operating theatre and at a low price, it is almost certainly an unauthorised substance. Its consequences are serious, permanent and only partly correctable. No particular volume is promised, and no promise is made that all the grafted fat will stay. These risks are reduced by the correct plane, prudent volumes and antithrombotic prophylaxis. They are not eliminated. The ones that apply to your case are set down in writing in the informed consent.

Common questions

The questions we are asked most about buttock augmentation

Performed badly, yes: it is the aesthetic operation with the highest mortality described, and the cause is known — injecting fat inside the gluteal muscle, which can cause a fat embolism.

Performed exclusively in the subcutaneous plane, with a blunt large-bore cannula and prudent volumes, its risk profile changes completely. That is why it is not done any other way here, even when that limits the volume.

Between a half and two thirds of what is grafted survives, as a rough guide. The rest is reabsorbed over the first three months, and that is allowed for in the planning.

That is why the buttock looks fuller at first and then comes down. In some cases a second session is considered from the sixth month onwards.

It changes the whole approach. Biopolymers produce chronic inflammation, migrate and distort the tissue, and they have no antidote. The first step is to assess with imaging what is there and where.

Removing them is complex surgery with a partial result, and it is not always possible to take them out completely. It is a consultation worth having even if the answer is not the one hoped for.

Directly on the buttock, around two weeks. After that you can sit for short periods using a cushion that rests on the thighs and keeps the grafted area free.

It is uncomfortable and it is the instruction that most influences how much fat survives: sustained pressure on the graft kills it.

Assessment consultation

The next step is a conversation about proportion and safety

How much fat you have available and what volume is safe to place are the two questions that define this surgery. You leave the consultation knowing what is possible in your case, what it involves and what it would cost, with no obligation to book anything that day.
