Indicative figures. Your own surgical and recovery times are set in the consultation, according to the technique indicated.

Surgery is performed in accredited clinics and hospitals in Lima, with the team and facilities each case requires. The consulting room is where you are assessed, where the plan is made and where follow-up happens.

The procedure

A breast lift removes excess skin and repositions the breast and the nipple at their natural height. It restores shape; it does not add volume.

With pregnancies, breastfeeding, changes in weight and the passing of time, the skin of the breast stretches and the tissue descends. The nipple ends up below the inframammary fold and the breast looks empty in its upper part. That is ptosis, and it is not corrected with exercise or with creams: skin that has stretched does not pull itself back.

The most frequent confusion in the consultation is between lifting and filling. A breast lift takes the tissue you already have and puts it where it should be; the result is a higher breast with a better shape, but the same size or even slightly smaller. If what you want along with shape is volume in the upper pole — the cleavage — then an implant or fat has to be added, and that is a different operation with different implications.

It usually helps when there is

  • A nipple sitting at the level of the inframammary fold or below it.
  • A breast that is empty in its upper part, with the volume displaced downwards.
  • Changes after pregnancy, breastfeeding or significant weight loss.
  • Stable weight, and plans for children completed or postponed.
  • A willingness to accept a permanent scar in exchange for the shape.

Worth discussing first if

  • A pregnancy planned in the near future: the changes can undo the result.
  • Current smoking, which greatly increases the risk to the skin and the nipple.
  • Looking mainly for volume: on its own, this surgery does not give it.
  • Unstable weight, or weight loss still under way.
  • A tendency to hypertrophic scars or keloids.

None of these rules the procedure out on its own; all of them change the conversation.

Options

The scar depends on how much has to be lifted

It is not chosen by preference: it is set by the degree of descent and the amount of skin to be removed.

Periareolar

A scar only around the areola. Suitable for mild descent. It corrects little and the areola can widen over time.

Vertical

Around the areola and a vertical line down to the fold. The one most used for moderate descent: good shape with a contained scar.

Inverted T

Adds a horizontal scar in the fold. Necessary when there is a lot of excess skin. It corrects the most and leaves the most marking.

Surgical judgement

Shape that lasts, not shape for the first month

The classic mistake in this surgery is to trust the shape to the skin. If the lift is held up only by tightening the skin, within a year gravity does its work and the breast descends again, with the nipple pointing upwards and the volume back at the bottom. That is why the tissue is reorganised and fixed deep down, and the skin is only closed over the top, without tension.

The nipple is moved while keeping its pedicle, that is, its blood supply and its nerves. The choice of pedicle depends on how far it has to be raised, and it determines both safety and the sensation afterwards.

And the position of the nipple is decided with the person standing, not lying on the table. Placing it too high is a mistake with no practical correction, so it is marked with a margin.

Stages

What the process is like

From the first consultation to discharge, with the usual timings for this surgery.

Consultation

The degree of descent, the quality of the skin and the current volume are measured, and the technique and the type of scar are decided. Whether volume needs to be added is discussed.

Before surgery

Laboratory tests, anaesthetic assessment and breast ultrasound or mammography according to age. Stopping smoking. The informed consent is signed.

Surgery

Under general anaesthesia. Marking while standing, removal of skin, reorganisation of the tissue, transfer of the nipple and closure in layers. A surgical bra at the end.

Follow-up

Review at one week, at three weeks, at the third month and at a year, with follow-up of the scars.

Timings

Week by week

The recovery is manageable; what takes time is for the shape to settle and the scar to mature.

  1. Days 1 to 7

Surgical bra at all times, moderate discomfort and limited ability to raise the arms. Swelling, and a very high shape: that is what is expected.

  2. Weeks 2 to 4

Back to office work at around ten to fifteen days. No exercise and no lifting. Any stitches that apply are removed.

  3. Weeks 6 to 12

Exercise built back up once authorised. The breast starts to settle and take its real shape. Active care of the scars.

  4. Months 6 to 12

Final shape, and scars in their last stage of fading. This is the point at which to judge the result.

Risks, and what is not promised

The general risks of surgery under general anaesthesia, and those specific to this one: scars that may widen, become hypertrophic or turn into keloids; asymmetry; altered or lost nipple sensation, which in a percentage of cases is permanent; problems with skin healing, above all in smokers; and, infrequently, partial or total loss of the nipple.

With time, and with further pregnancies or changes in weight, the breast descends again: the surgery corrects the current state, it does not stop the process. And the volume of the upper pole tends to reduce over the first months, something that is anticipated when planning.

On breastfeeding: most techniques preserve the possibility, but it cannot be guaranteed. These risks are reduced by a technique that fixes the tissue deep down, closure without tension and giving up smoking. They are not eliminated. The ones that apply to your case are explained to you in the consultation and are set down in writing in the informed consent.

Common questions

The questions we are asked most about breast lift surgery

No. A breast lift repositions what you already have and usually leaves the breast the same size or a little smaller, because skin and sometimes a little tissue are removed.

If what you want is volume in the cleavage, an implant or fat has to be added. That is a different operation, and it is better decided in the consultation than after surgery.

They are permanent. Over time they fade and flatten until they are discreet in most cases, but they do not disappear, and on some skin they stay more marked.

That is the real trade-off of this surgery. The shortest scar the case allows is always chosen, although what decides which one is possible is the degree of descent, not preference.

In most cases yes, because the usual techniques preserve the connection between the nipple and the glandular tissue. But it cannot be guaranteed.

If you are planning a pregnancy soon, the sensible thing is to wait: pregnancy and breastfeeding change the breast again and can undo part of the result.

Years, and it depends a great deal on the quality of your skin, on your weight and on whether there are later pregnancies. The breast goes on ageing after surgery.

What the surgery does is give you back a much better starting point. What helps most to keep it is a stable weight and a good bra when exercising.
