Indicative figures. Your own surgical and recovery times are set in the consultation, according to the technique indicated.

Surgery is performed in accredited clinics and hospitals in Lima, with the team and facilities each case requires. The consulting room is where you are assessed, where the plan is made and where follow-up happens.

The procedure

It combines lifting the breast with placing an implant. It resolves both the descent and the lack of volume at the same time, which is the most common situation after pregnancies.

It is the most frequent request in the breast consultation: the breast has dropped and has also emptied at the top. A breast lift on its own restores the shape but leaves the cleavage flat; an implant on its own fills, but in a sagging breast it worsens the descent by adding weight. The combination solves both.

It is also, by some distance, the most demanding breast operation, and it is worth knowing why. The two manoeuvres pull in opposite directions: the lift reduces and tightens the envelope of skin, while the implant expands it from inside. That conflict is why this surgery has higher reoperation rates than either of the two separately. It is not a reason not to do it; it is a reason to plan it with moderate implants and to raise the option of two stages when the case calls for it.

It usually helps when there is

  • A breast that has descended and is also empty at its upper pole.
  • Changes after pregnancies, breastfeeding or weight loss.
  • A nipple below the inframammary fold with little tissue of your own.
  • Stable weight, and plans for children completed or postponed.
  • A willingness to accept scars and an implant that will need monitoring.

Worth discussing first if

  • Wanting a large implant on poor-quality skin: it is the highest-risk combination.
  • Current smoking, which compromises the skin and the nipple in an already demanding operation.
  • Very marked descent, where operating in two stages may be preferable.
  • A pregnancy planned in the near future.
  • Expecting a result with no scars or no follow-up.

None of these rules the procedure out on its own; all of them change the conversation.

Options

In one stage or two

The decision is not about convenience: it depends on how much skin has to be removed and on which implant is needed.

In a single stage

The usual approach. The lift and the implant are done in the same operation. It needs a moderately sized implant and skin of reasonable quality.

In two stages

The lift first and, a few months later, the implant. It is proposed for very marked descent or fragile skin: it lengthens the process and greatly reduces the risk of complications.

With your own fat

For those who prefer not to carry an implant and need little volume, the lift is complemented with a fat graft. It gives less projection and needs no replacements.

Surgical judgement

A moderate implant is the key

The rule that most protects the result in this surgery is not to be ambitious about volume. A large implant on skin that has just been tightened is the recipe for wound breakdown, early descent and a nipple that ends up pointing upwards. The implant is chosen for what the skin and the chest will tolerate, not for the size wanted.

The sequence matters too: the implant goes in first, the shape is checked with the person sat up on the table, and only then is it decided how much skin to remove. Doing it the other way round leads to closures under tension.

And there are cases where the right answer is to split the surgery in two. It is an uncomfortable conversation — it means two anaesthetics and two recoveries — but it is preferable to a complication that forces the implant to be removed.

Stages

What the process is like

From the first consultation to discharge, with the usual timings for this surgery.

Consultation

The degree of descent, the quality of the skin and the volume wanted are assessed, and whether the case calls for one stage or two is decided. The implant is chosen.

Before surgery

Laboratory tests, anaesthetic assessment and breast imaging. Stopping smoking. The informed consent is signed.

Surgery

Under general anaesthesia. Marking while standing, placement of the implant, checking the shape sitting up, removal of skin and closure in layers. The implant card is handed over.

Follow-up

Reviews at one week, three weeks, the third month and a year, and then periodic reviews because of the implant.

Timings

Week by week

It is somewhat more demanding than either of the two operations on its own.

  1. Days 1 to 7

Surgical bra at all times, moderate discomfort and a feeling of pressure. Breasts very high and firm: that is what is expected.

  2. Weeks 2 to 4

Back to office work at around twelve to eighteen days. No exertion and no arms above the shoulders.

  3. Weeks 6 to 12

Exercise built back up once authorised. The implant settles, the breast softens and the real shape appears. Care of the scars.

  4. Months 6 to 12

Final shape and scars in their last phase. This is the point at which to judge the result.

Risks, and what is not promised

It adds up the risks of both operations. From the lift: scars that may widen or become hypertrophic, asymmetry, altered or lost nipple sensation, healing problems and, infrequently, loss of the nipple. From the implant: capsular contracture, rupture, displacement, visible edges, late seroma and the need for replacement or removal at some point in life.

And it adds a risk of its own: the reoperation rate is higher than for either operation separately, because the two manoeuvres work in opposite directions. An adjustment operation may be needed, and that is said beforehand, not afterwards.

On anaplastic large cell lymphoma associated with implants: it is a very rare entity, linked above all to textured surfaces, which is explained in the consultation with the current data. No particular result is promised, and no implant is promised for life. These risks are reduced by moderate implants, closure without tension and giving up smoking. They are not eliminated. The ones that apply to your case are set down in writing in the informed consent.

Common questions

The questions we are asked most about breast lift with implants

Because in your case lifting and filling at the same time would put the skin at risk: a lot of tension on a fresh closure, with the possibility of the wound opening or the nipple suffering.

Splitting it means two anaesthetics and two recoveries, and it costs more in time. In exchange, it greatly reduces the chance of a complication that would force the implant to be removed.

The implant chosen is the one your skin and your chest will tolerate, and in this operation in particular that margin is narrower than usual.

A large implant on freshly tightened skin is the most frequent cause of complications and reoperations here. It is the conversation that is sometimes awkward in the consultation, and it is the one that protects your result.

Yes, and it is worth knowing beforehand. Combining two opposing manoeuvres makes fine adjustment harder, and a proportion of patients need a second, smaller operation.

That is not a reason to rule it out: it is a reason to plan it prudently and not to promise that a single operation will resolve everything.

No. They are devices with a working life and at some point they may need replacing or removing, because of rupture, contracture or simply a change of mind.

You are given the card with the make, model and batch number, and a plan of reviews is set out. Going into this surgery means taking on long-term follow-up.
