The treatment

Biostimulators do not fill: they prompt your skin to produce new collagen. The result is an improvement in the firmness and thickness of the skin that appears over months and lasts longer than a filler.

The difference from hyaluronic acid is fundamental and worth understanding before deciding. A filler occupies a space and shows the same day; a biostimulator is a particle the body recognises and around which it manufactures collagen over weeks. That is why the effect takes two to three months to become evident, and why it also lasts considerably longer.

There is a practical consequence that is almost never mentioned: biostimulators cannot be dissolved. Hyaluronic acid has an antidote, hyaluronidase, which makes it possible to reverse a result you did not like or to treat a complication. Here that resource does not exist. It is the reason why the dose is always kept on the low side and adjusted over successive sessions.

It usually helps when there is

  • Skin that has lost firmness and thickness, rather than volume in one spot.
  • Mild or moderate laxity of the face, neck or décolletage.
  • A general loss of support, with no particular area to fill.
  • Skin of the arm, abdomen or thigh with mild laxity.
  • Wanting a progressive result that those around you do not really notice.

Worth discussing first if

  • Marked laxity: biostimulators do not replace surgery.
  • Expecting an immediate change: there is none here.
  • A history of nodules or granulomas with previous injectables.
  • Active autoimmune disease or infection in the area.
  • Current pregnancy or breastfeeding.

None of these rules the procedure out on its own; all of them change the conversation.

Products

They do not all do the same thing

They are chosen according to the area, the depth and how much support is needed. All of them must hold a current health licence and be applied from their original packaging.

Poly-L-lactic acid

The most gradual biostimulator. It needs two or three sessions weeks apart and its effect builds over months. Widely used on the face and neck.

Calcium hydroxylapatite

Gives a support effect that is somewhat more visible from the start, and also stimulates collagen. Useful along the jawline, the neck and the décolletage.

Polynucleotides and similar

They work on the quality and hydration of the skin more than on support. They are combined with the above in thin or sun-damaged skin.

Medical judgement

A low dose and patience

The golden rule with biostimulators is to under-dose. Because the effect takes months to complete, anyone injecting with tomorrow's result in mind ends up putting in too much, and the excess shows up three months later as firm areas or nodules that cannot be dissolved. Less is applied than seems necessary, and it is reassessed at two months.

Technique matters too: properly diluted product, even distribution, the correct plane and massage afterwards as the manufacturer directs. Nodules almost always come from product concentrated in one spot, not from the product itself.

And one precondition: because there is no antidote, this is applied by a doctor, with a product holding a current health licence and from its original packaging. It is exactly the same standard that governs fillers, only here the margin for error is smaller.

Stages

What the treatment is like

It is almost always a series of sessions, not a single one. The plan is set at the first consultation.

Consultation

The quality of the skin and the degree of laxity are assessed, and whether the case calls for biostimulation, filler or surgery. Product and number of sessions are defined.

Before the session

Stopping anti-inflammatories where possible and avoiding alcohol the day before. The informed consent is signed.

The session

In the consulting room, with topical anaesthesia. The product is shown to you in its packaging before it is applied. Massage over the following days is prescribed at the end.

Later sessions and review

Second and third sessions every four to six weeks depending on the product, with a review at three months to judge the real result.

Timings

How it progresses

The visible part lasts hours; the part that matters takes months.

  1. First 48 hours

Redness, some swelling and occasionally bruising. Massage of the area as directed, and avoiding intense exercise.

  2. Weeks 1 to 4

The skin looks the same as before. That is normal and does not mean the treatment has not worked: the collagen is still forming.

  3. Months 2 to 3

Better firmness and texture start to show. This is the point at which to judge and decide whether another session is needed.

  4. Months 6 to 24

Maximum effect, then a plateau. The result is gradually lost and maintenance is considered according to how things go.

Risks, and what is not promised

What is usual is mild: redness, swelling, bruising and tenderness at the injection points for a few days. Less frequent but relevant is the appearance of palpable nodules, which can emerge weeks or months later and which, with no antidote available, need treatment with injections, massage or even minor surgery.

As with any facial injectable, there is the infrequent but serious risk of vascular occlusion. With these products the situation is more difficult than with hyaluronic acid, precisely because they cannot be dissolved. On leaving you are told which symptoms mean calling immediately: severe pain, pallor or a change in skin colour, disturbance of vision.

No particular result is promised, and no permanent effect either: the response depends on your own capacity to produce collagen, which varies with age, genetics and smoking. These risks are reduced by conservative doses, licensed product and medical application. They are not eliminated. The ones that apply to your case are explained to you in the consultation and are set down in writing in the informed consent.

Common questions

The questions we are asked most about biostimulators

A filler occupies space and shows the same day; a biostimulator makes your skin manufacture collagen and shows at two or three months. One gives volume in a particular spot, the other improves firmness and thickness across the board.

And there is a practical difference worth keeping in mind: hyaluronic acid can be dissolved with an antidote; biostimulators cannot.

Usually two or three, four to six weeks apart, although it depends on the product and on the starting point. With calcium hydroxylapatite one is sometimes enough.

The number is adjusted as you go: it is assessed at two or three months and then decided, rather than sold as a fixed package up front.

For mild laxity, yes: they improve the thickness and firmness of the skin, and in those areas it shows. For moderate or marked laxity, no: they do not lift tissue.

When the problem is excess skin, no injectable removes it. There the conversation is about surgery, and it is better to have it than to spend on sessions that will not get there.

It is treated. Depending on the type and the timing, it is managed with massage, with a local injection or, in isolated cases, with a small excision. Most resolve.

What matters is that you raise it as soon as you notice it rather than waiting months: the sooner it is treated, the simpler it is.

Assessment consultation

The next step is seeing what your skin needs

Sometimes it is biostimulation, sometimes filler, sometimes both and sometimes neither. A consultation clears it up. You leave knowing what is possible in your case, what it involves and what it would cost, with no obligation to book anything that day.
